Total Bilirubin () is the sum of conjugated and unconjugated serum bilirubin. One of the byproducts of red blood cell breakdown is bilirubin.  Bilirubin is made in the bone marrow, liver, or spleen and is transported by albumin to the liver in the form of unconjugated bilirubin.  Once in the liver, sugars bind to the unconjugated bilirubin, turning it to conjugated bilirubin. It is then excreted from the gall bladder into the small intestine. Eventually, it is excreted in the feces or urine.  Excess bilirubin causes a yellowing of the skin and the whites of the eyes called jaundice.

It is currently thought that Cogan’s syndrome is an autoimmune disease. The inflammation in the eye and ear are due to the patient’s own immune system producing antibodies that attack the inner ear and eye tissue. Autoantibodies can be demonstrated in the blood of some patients, and these antibodies have been shown to attack inner ear tissue in laboratory studies. Infection with the bacteria Chlamydia pneumoniae has been demonstrated in some patients prior to the development of Cogan’s syndrome, leading some researchers to hypothesis that the autoimmune disease may be initiated by the infection. Chlamydia pneumoniae is a common cause of mild pneumonia, and the vast majority of patients who are infected with the bacteria do not develop Cogan’s syndrome.

Columbia University is committed to establishing and maintaining a healthy and safe work environment for our staff, students, neighbors and surrounding communities. Through the recognition, evaluation, and control of personal and environmental hazards, the University strives to eliminate individual risk and reduce the environmental impact of its activities. EH&S offers a broad range of services and actively develops partnerships with faculty and departmental personnel to ensure a safe work environment and compliance with University policy and applicable regulations in the most efficient manner possible. These endeavors are realized through programs such as personnel training, chemical hygiene plan, biological safety, environmental safety, fire safety, occupational safety, and asbestos and lead management in compliance with local, state and federal regulations.

LDH can also reflect blood acidity and pancreatic function as well as being an intercellular enzyme prominent in heart and skeletal muscles. Persons on nucleoside analogues (AZT, DDI, DDC) who exercise frequently commonly have an elevated LDH from muscle tissue breakdown. Others who do not exercise, but are on these drugs -or are wasting- may also have an increased LDH. Persons with Pneumocystis pneumonia tend to have a more serious prognosis if they have an elevated LDH. Decreases indicate an alkaline tendency to the blood. A high LDH often indicates that a heart attack has recently occurred. It can also be high during active cancer growth, however it cannot be considered a specific cancer indicator.
